Washington State requires families to provide immunization documentation for children participating in our preschool program. Use the resources below to access your child's immunization information and complete the appropriate form.
1. Access Your Child's Immunization Records
Need a copy of your child's immunization history? Access your family's records online before completing the required form.
2. Complete the Immunization Form
Complete the required immunization form using your child's immunization information.
NOTE: Immunization records MUST be submitted using the required form. Another printout or other record of your child's immunizations does NOT replace the completed form at the button above.
